On the day of your procedure, you’ll lie down on a comfortable treatment table and be given protective eyewear. Your provider will use the Excel V+ handpiece to deliver a few rapid pulses of light to the targeted area. You might feel a cooling sensation on your skin, a feature of the Excel V+ system designed to enhance comfort and protect the skin’s surface. 

The sensation of the laser pulse is often described as a quick, mild sting, comparable to the snap of a rubber band against the skin. The duration of the session varies depending on the area’s size and the number of veins being treated, but many treatments can be completed in under 30 minutes.

Recovery

Recovery from laser vein therapy is typically quick. Patients can usually resume normal activities the same day, although it’s advised to avoid strenuous exercise for a short period. Compression stockings may be recommended to support healing, but most of the time they’re not necessary. Mild bruising or swelling can occur but should subside within a few days.

Endovenous laser therapy, sclerotherapy, and other more invasive treatment options like vein stripping and phlebectomy offer different approaches to treating varicose and spider veins. Here’s how they work:

Endovenous laser ablation

Endovenous laser therapy (EVLT) involves first making a small incision in the targeted area and routing a laser fiber through a small catheter. As with non-invasive laser vein therapy, the laser light causes the vein to close and blood to reroute to healthier veins. This method is ideal for patients with small to medium-sized varicose veins and spider veins. It’s an outpatient procedure performed by a vascular surgeon under local anesthesia with minimal discomfort.

Radiofrequency ablation

Radiofrequency ablation (RFA) uses radiofrequency energy to heat and subsequently close off the malfunctioning veins that lead to varicose veins. It’s performed in a similar manner to EVLT: a small catheter is inserted into the vein through a tiny incision and emits radiofrequency energy, heating the vein walls and causing them to collapse and seal shut.

Sclerotherapy

Sclerotherapy involves injecting a solution directly into the vein, causing it to scar and reroute blood to healthier veins. This treatment is effective for smaller varicose veins and spider veins. Compared to laser therapy, sclerotherapy may be more effective for certain types of spider veins and smaller varicose veins but might require multiple sessions.

Vein stripping and ligation

Vein stripping and ligation is a surgical procedure that involves removing large varicose veins through small incisions. This method is generally reserved for severe cases with large, bulging varicose veins. The procedure requires general or spinal anesthesia and has a longer recovery period than laser therapy or sclerotherapy. Patients who have not responded to less invasive treatments or have very large varicose veins might consider this option.

Ambulatory phlebectomy

Ambulatory phlebectomy involves making small punctures to remove smaller varicose veins close to the skin’s surface. This procedure is less invasive than vein stripping but more invasive than laser therapy or sclerotherapy. It’s typically performed under local anesthesia and is suitable for patients with bulging varicose veins that are too large for laser therapy or sclerotherapy but not severe enough to require vein stripping.
